Your Partner in Achieving Health and Wellness at Home

Home Health Care is one of the fastest growing industries in the nation today. This is caused by an increased elderly population, the prevalence of chronic illnesses and increasing hospital costs.

As part of Care for Life Home Health, Inc. dedication to continuously promote improvement in quality, efficiency and outcomes for Medicare beneficiaries, we have redesigned our care delivery to promote more effective, efficient and high quality care processes. This is an important step in how the Centers for Medicare and Medicaid Services (CMS) is moving towards achieving better value, outcomes and patient-focused care instead of the volume of services provided.

Our goal is to provide a value based agency and not just quality outcomes. We are more than reducing re-hospitalizations but focus on using evidenced-based practice because we believe that it is the “Standard of Care”.

Heal at Home™ Program specializes in the evaluation and treatment of acute and chronic, non-healing wounds resulting from Diabetes, Peripheral Arterial/Vascular Disease, post surgical complications which can be exacerbated by obesity, poor nutrition and infection. As the aging population grows, so is the risk of developing pressure ulcers.

  • Three million Americans experience venous leg ulcer which is usually caused by disease veins.
  • Pressure ulcers in the elderly is associated with a fivefold increase in mortality and a fifth of pressure ulcers develop at home.
