Caring for patients with chronic wounds is an increasing problem and choosing a provider that recognizes and address the needs of these patients is so vital. Wound care patients often have co-morbidities and chronic conditions. As health care evolves towards a patient Centered Care Model, a comprehensive treatment that extends beyond the wound itself ensures an effective wound management and prevention. Studies showed that patient outcomes are better when care is provided in an environment of focused expertise. Heal at Home™ specializes in the evaluation and treatment of acute and chronic non-healing wounds resulting from Diabetes, Peripheral Arterial/Vascular disease, post surgical complications which can be exacerbated by obesity, poor nutrition and infection. As the aging population grows, so is the risk of chronic wounds. We certainly understand the seriousness and challenges of the above conditions so we develop strategies of care that are proven to work and implemented individualized treatment options as well as general wound management principles applicable to all varieties of chronic wounds.
INFUSION THERAPY
Home Infusion Therapy is an excellent alternative for patients to receive infusion therapy services in the comfort of their homes. Our home infusion Registered Nurse is familiar with all types of peripheral and central lines for intravenous infusion and performs a comprehensive assessment with each nursing visit. The nurse also trains the patient or caregiver on how to properly administer medications at home, monitors the patient’s progress throughout the therapy and reports progress or complications to the ordering physician. The Home Infusion Pharmacist also performs a thorough assessment and helps identify any potential issues that can adversely affect the patient. They also deliver the medication and all the necessary supplies for infusion directly to the patient’s home. CFLHH reviews a patient’s insurance benefits prior to infusion therapy. The patient and caregivers are informed of coverage and any financial responsibility.
DISEASE MANAGEMENT
One of the most effective ways to prevent hospital readmission is to manage chronic health conditions. According to the CDC, more than 80% of adults age 65 and older have at least one chronic condition, and 50% have multiple chronic conditions. Care for Life Home Health, Inc. is committed to deliver care programs that are preventive, holistic and personalized. This may include a treatment plan with regular monitoring, coordination of care between multiple provider and or settings, medication management, evidence-based care, measuring care quality and outcomes, and support for patient self-management, through education or tools.
REHABILITATION & THERAPY SERVICES
CFLHH interdisciplinary team of physical, occupational and speech therapists and Home Health Aide work closely with physicians to follow the recommended course of therapy and assess patient’s progress. Receiving rehabilitative therapies at home is a safe, convenient and effective alternative to placement in a rehabilitation hospital or nursing home. By allowing the patient to remain independently provides added motivation to follow the prescribed course of therapy and return to optimal levels of functioning.
